Congenital muscular torticollis is usually discovered in the first 6 to 8 weeks of life. It is characterized by a contracture of the sternocleidomastoid muscle, which causes the head to flex laterally towards the affected side and turn towards the opposite shoulder. If left uncorrected, this posture can lead to plagiocephaly and upper thoracic scoliosis.

Specially designed for the treatment of congenital muscular torticollis, the TOT Collar™ is a simple and effective therapeutic product that maintains a correct position of the head, preventing future problems. It is normally used in cases of infantile torticollis, but it can also be used in adults.

The TOT Collar™ is designed to provide a noxious stimulus to the lateral aspect of the skull. The user moves away from this stimulus towards a new corrected central position. Adopting a new normal head position provides the ability to reestablish the perception of horizontal and thus maintain the corrected head position.

The use of the TOT Collar™ is added to the conservative treatment of babies with congenital muscular torticollis if they are 4 months of age or older and show a constant head tilt of 5 degrees or more. The baby must have adequate ROM (range of motion) and lateral head straightening reaction (head control and strength) to lift the head away from the side of the collar.

A soft PVC tubing loop is bridged by two short nylon tubing. The necklace includes different sizes of tubes. These tubes are placed anterior and posterior to the crest of the trapezius. The PVC pipe is fastened with a connecting strap. A replacement velcro strap is included.


The TOT Collar™ is non-toxic and washable.

The TOT Collar™ is preassembled and ready to fit. There are two sizes: adult and child. The clear PVC tube of the Collar is supplied at twice the circumference of the neck, plus 4 ”-6” (10cm-15cm). The tube is attached with an end connector.

White tubes or struts are selected according to the amount of head tilt. Additional tubes are supplied with the TOT Collar™ to allow for change in lateral tilt.

  • Tube A extends from posterior to the crest of the trapezius to the occiput.
  • Tube B from anterior to the crest of the trapezius to below the tip of the mastoid process. Avoid pressure on the jaw.

The tubes are short at first and are replaced by longer tubes as the correction is achieved. If there is a significant slope, initially use very short tubes. Tubes can be trimmed to the exact length required.

For a correct fit, the baby should hold their head in the midline, slightly away from the struts, and there should be room for at least one adult finger to fit between the upper clear pvc tube and the neck.

At first, fit the collar loosely and make sure the side tubes are the correct length and on the correct side. Adjust the neck by hand to check the fit.

Take the necklace off the child. Trim this tube to length with scissors and reconnect it to the end fitting. Replace the collar after each cut to verify fit.

Indications: The TOT Collar™ is used in the conservative treatment of children with congenital muscular torticollis.

Contraindications: excessive compression can cause discomfort. The time of use must be prescribed by the doctor. If problems of skin irritation, sensitization and cytotoxicity are observed, remove it and consult a doctor.
